APPLICATION FOR MEMBERSHIP OF UVPERSU

I, the undersigned, hereby apply for membership of the above-mentioned union.

This application is subject to approval by the Management Committee of UVPERSU.

With this application I abide by the terms of the constitution of UVPERSU.

DEBIT ORDER: UVPERSU MEMBERSHIP

I hereby authorise the Finance Department (Salary division) to deduct an amount of R45 monthly from my salary for payment to UVPERSU.  This debit order also applies when I am on unpaid leave.  This authorisation can be cancelled via the UVPERSU office, with ONE month's written notice.
